The 45th Sustainment Brigade's Shoulder Sleeve Insignia, or unit patch, was approved on 12 January 2006, one day after the unit was redesignated from the 45th Corps Support Group (Forward) and assigned two Support Battalions and a Special Troops Battalion. Six months later, the Brigade was deployed in support of Operation Iraqi Freedom, with the Special Troops Battalion earning a Meritorious Unit Commendation for providing Signal, Medical, Logistical, and Personnel service support for Multi-National Division North between July 2006 and June 2007.
Two more deployments would follow in 2009 and 2012, earning the Brigade two campaign participation credits for Operation Enduring Freedom. In 2015, the Brigade was redesignated the 25th Sustainment Brigade and assigned to the 25th Infantry Division.
Shoulder Sleeve Insignias are offered as Color Patches or ACU (Army Combat Uniform) Patches. Color patches are designed for wear on the shoulder of the Class A/B Army Green Uniform, which was phased out in 2015; the modern equivalent is the Combat Service ID Badge (CSIB—link below) worn on the Army Service/Dress and Mess/Evening Mess uniforms.
ACU patches are offered in two patterns, UCP (Universal Camouflage Pattern) and OCP (Operational Camouflage Pattern). The UCP was the original camo pattern used for the ACU and other camo utilities, but production of UCP-based uniforms ceased in 2015 and uniforms featuring that pattern were given a wear-out date of 30 September 2019.
