Commonly referred to as a unit crest or as a DUI for short, the 1st Medical Brigade Distinctive Unit Insignia was originally approved for the 1st Medical Regiment on 19 December 1923. It was redesignated (and amended to include the motto “Fortitude And Compassion”) on 20 March 1968 for the 1st Medical Group, with second amendment to correct the symbolism on 26 April 1968. It was given its current designation on 6 June 2000.
The insignia consists of a maroon shield and a silver scroll enveloping all but the top of the shield. On the shield, a wreath of silver oak leaves encompasses a silver knight’s helmet upon which a red numeral “1” is emblazoned, a reference to the Shoulder Sleeve Insignia of the 1st Division with which the unit served in the First World War. A helmet is a reference to the military aspect of the organization.

Distinctive Unit Insignias are worn by all Soldiers (except General Officers) in units that have been authorized to be issued the device. It is worn centered on the shoulder loops of the Army Green Service Uniform (AGSU) and the blue Army Service Uniform (ASU, Enlisted only) with the base of the insignia toward the outside shoulder seam. DUIs are not worn on the Dress variations of either uniform, however.
More guidance on wear of the DUI is found in DA Pamphlet 670-1, Section 21-22, "Distinctive unit insignia" and 21–3(d) and (e), "Beret" and "Garrison Cap," respectively.
