The two stars surmounting a fouled anchor reflect the E-9 rate of Master Chief Petty Officer (MCPO), two above the E-7 rate of Chief Petty Officer—the highest enlisted rank in the Coast Guard until Congressional legislation in 1958 authorized two higher pay grades.
Large MCPO cap devices are worn on the Combination Cap and Cold Weather Cap. The Small version is worn on the Garrison Cap, where it’s placed on the wearer’s left side two inches from the front crease with its center one-and-a-half inches from the bottom of the cap. Small MCPO insignias sold in pairs also serve as the collar insignia for Light Blue and Winter Dress Blue Shirts, as well as the Windbreaker and Trench Coat.
