A Commissioned Officer in the O-2 grade is designated as a Lieutenant, Junior Grade (LTJG) in the Navy and Coast Guard and a First Lieutenant (1stLt) in the Marine Corps and is required to wear insignia of grade on the Garrison Cap. In the Navy, LTJGs wear Garrison Caps almost exclusively with the Service Khaki uniform, but they are also authorized for wear with the Service Dress Blue (SDB) uniform when the black relax-fit jacket and/or black V-neck sweater are worn in place of the SDB coat. Coast Guard ensigns wear Garrison Caps with the Tropical Blue, Winter Dress Blue, and SDB “Bravo” uniforms, while male and female First Lieutenants in the USMC wear Garrison Caps with the Service Uniform (Commanders may prescribe the Service Cap in its place, however).
The regulations for all three services call for the cap rank insignia to be worn on the wearer’s right side of the cap. USMC First Lieutenants position the insignia so that the cap’s equally bisects the insignia, while LTJGs place the insignia so its center is two inches from the cap's front crease and one-and-a-half inches from the bottom of the cap. All three services specify that insignia is to oriented so the long axis of the silver bar is vertical.
