4th Psychological Operations Group (Airborne) was constituted in November 1967 and activated on 1 December 1967. It was inactivated for less than a year in 1971 and 1972 before being reactivated on 13 September 1972 at Fort Bragg, still the group's home but now named Fort LIberty. In addition to its Headquarters Company, the Group comprises three Psychological Operations (PSYOP) Battalions providing support for Europe (6th), Africa (7th), and the Middle East (8th), as well as a Dissemination Battalion (3rd) responsible for providing various types of media (print, broadcast, audio, etc.).

Approved on 8 December 1986, the organizational flash and oval for the 4th Psychological Operations Group use some of the design elements from its Shoulder Sleeve Insignia (unit patch). The white, gray, and black areas represent the three types of propaganda: truth, half-truth, and untruth. Three lightning flashes denote the three primary methods of disseminating PsyOp messaging that were being utilized when the unit patch was approved in 1968: audio, visual, and direct personal persuasion.
