Lieutenants in the U.S. Navy and Coast Guard share the same pay grade (O-3) as Captains in the Army, Air Force, and Marine Corps, but only the latter uses the same size metal rank insignia for wear on hats, collars, and coats as the two seagoing services. Navy Lieutenants wear the two silver Lieutenant bars centered between the front and rear edges of the shoulder straps on All-Weather Coats and Blue and Black Jackets, with the outer edge of the bar 3/4-inch from the squared of the strap; the insignia is oriented so the bars are in a “fore and aft line,” i.e., the long axis points forward and backward. Lieutenants in the Coast Guard follow the same wear guidance as their USN counterparts, but they wear metal rank insignia only on the epaulets of the Windbreaker and Trench Coat.
Marine Corps regulations on the placement of coat rank devices is almost identical to the Navy’s. Shoulder grade insignia is placed so it is equidistant from the rear and front edges of the shoulder straps on Dress jackets and Dress/Service coats, the Tanker Jacket, All-Weather Coat, and Field coat. Like in the Navy and Coast Guard, devices are placed 3/4-inch from the squared (shoulder) end of the strap.
