As an all-Officer service, the United States Public Health Service Commissioned Corps issues a single style of cap device for its personnel. It features a shield containing 13 stars and stripes mounted on the USPHS emblem of a crossed fouled anchor and caduceus, surmounted by an eagle pointing to the wearer’s right.
The USPHS CC cap device is manufactured in two sizes, with the large of full-size version worn on the Combination Cap by attaching it to the mounting area in front. For wear on the Garrison Cap, the device is worn in tandem with a miniature metal rank insignia—rank on the right, miniature cap device on the left—with both centered 1.5” from the lower edge of the cap and 2 inches from the front crease. The device is placed on the beret so it’s aligned over the wearer’s left eye.
The current version of the USPHS CC was introduced in 2012 as part of the service’s efforts to bring Mil-Spec standards to all its insignias, badges, and other accoutrements.
